Nomination Procedures
All nominations must be submitted on a nomination form and be accompanied by an explanation of why you are nominating this individual or organization.
Please
provide a (1 to 2 page) submission outlining the nominee’s key accomplishments according to the
following headings:
- a) contributions to heart health within the specific award category;
- b) explanation of how the nominee is a heart healthy role model;
- c) notable and unique achievements in heart health.

(If applicable, you may use point form with specific examples and dates.)
All nominations must be received no later than February 25, 2005 and must include:
- the nomination category;
- you and your nominees’ names, addresses, phone numbers, and signatures;
- at least one other reference contact for the nominee;
- supporting information for your nomination.
